RELEASE: I understand that my presence, activity, and participation in classes and other related activity at Shotokan Karate Minnesota is completely at my own risk; and I hereby release and discharge Shotokan Karate Minnesota and all instructors, employees, heirs, and assigns of those entities heretofore mentioned, from all claims, demands, expenses, losses, and causes of action of every kind whatsoever involving damages of every kind, nature, or description which may hereinafter arise from or out of any injuries (including deaths and/or illnesses), or alleged injuries to persons, including injuries which I may receive or perpetrate sustained directly or indirectly, out of the operation of classes and other related activities at/of Shotokan Karate Minnesota MEDIA: I understand that any video, photos, images of my child taken during class, and quotes or other written materials may be used by Shotokan Karate MN LLC or assigned agents for art, advertising, and promotional literature. Guardians waive the right to inspect and/or approve the finished product or copy. We will never use names or contact information of students on any of our media materials or promotions.
